Forms Required for Fall 2010 Registration:


The Fall 2010 registration dates have passed.  Intramural (U-6 and U-8) registrations can still be sent in the mail to: Panthers USA, PO Box 3155, York, PA 17402.  U-6 and U-8 fees are still $60, late fees do not begin until June 1.  Travel (U-9 and older) players may not mail in registrations any more.  All required forms and a check (including late fee) should be brought to the first tryout date for your age group.  The tryout Administrators will not be able to make change, so if paying in cash please have the exact amount ($95 which includes the late fee).

 

First determine the age group of your child on the Age Group Chart for the Fall 2010 through Spring 2011 Season.

 

The Panthers USA Intramural (in-house) program is for players up through U-8.  Experienced players at U-8 may wish to play U-9 travel.  All players U-9 and older will all play travel, as the club does not have enough players for our in-house league at these ages.

 

Players registering for the Intramural Program need items #1, #4 and possibly #6 below.

Travel players need all items #1-5 and possibly #6.

2. EPYSA RG-6 (required for all travel players)

3. Players Code Of Conduct (required for all travel players)

4. Parents Code Of Conduct (at least one parent of every player)

5. A Medical Release Form (travel players only)
6. Players that are new to the club will need to provide a copy of their birth certificate

 

All players U-9 and older should plan to attend tryouts.  In order to be considered for the first team in an age group, the player must attend at least one of the tryouts of that age group.  Players that are not interested in the first team do not need to attend tryouts, but are welcome to participate if they want.  All tryouts will be held at Sinking Springs Elementary School starting April 19 and running through early May.
